Had the same problems and similar troubleshooting as the OP (installing the 4GB patch, stutter remover etc.). Then I followed the Viva New Vegas guide without including any uneccessary mods and now the game runs perfectly. I definitely recommend giving it a try: Stutter Remover causes crashes directly, remove it.
